The Ultimate Health program provides medical reimbursement insurance intended to supplement your primary medical coverage. Certain expenses incurred by you and your eligible dependents are qualified for reimbursement:

Per Occurrence Limit: $5,000 for those enrolled in an HMO, HDHP or PPO

Eligible Expenses: Unreimbursed medical, dental, and vision care expenses not covered by your health plans.

Examples of Eligible Expenses: Deductibles, coinsurance, copayments for office visits and prescription drugs, eye care materials, chiropractic care.
